Physical Therapist Home Health. This position provides skilled home health services in patients homes, managing an individual patient caseload and completing required documentation. The assignment includes day shifts, approximately 30 productivity points per week, and up to six patient visits per day.
Job Details Required
At least 1 year of skilled home health experience; OASIS experience; Home Care Home Base (HCHB) experience; Basic Life Support (BLS) certification; active Oregon registered nurse (RN) license required before the start date; reliable personal transportation; active drivers license; timely communication and documentation.
Day shifts, 8-hour shifts, 40 hours per week. Additional Information: Skilled home health setting with a 1:1 patient ratio. Weekend support may be required, with no routine call requirement.
First-time travelers are accepted. Business-casual attire is required; scrubs are permitted but not required. Documentation is completed using Home Care Home Base (HCHB), a remote home health client, and a mobile application.
The assignment start date is 11/30/2026. Orientation is scheduled for Monday.
