Travel Physical Therapist (PT) – Home Health White River Junction, Vermont We are seeking a Travel Physical Therapist for a 16-week Home Health contract in White River Junction, Vermont . Position Highlights Home Health Setting 16-Week Travel Contract Full-Time Schedule Day Shift Adult & Geriatric Patient Population One-on-One Patient Care Independent Field-Based Role White River Junction, Vermont Responsibilities Perform comprehensive physical therapy evaluations and reassessments. Develop individualized treatment plans based on patient needs and physician orders. Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education. Educate patients and caregivers on home exercise programs, safety strategies, and equipment use. Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
Complete timely documentation and maintain home health compliance standards. Manage a home health caseload while self-scheduling visits based on patient needs. Why White River Junction?
