Guaranteed Hours 0 hours Productivity 30 units Required Qualifications HCHB experience required. OASIS experience required. Home Health experience required.
Skills & Experience Current state license as a Physical Therapist - Required. Current CPR certification - Required. Current driver's license - Required.
Reliable transportation and valid auto insurance - Required. 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required. Previous home care experience - Preferred. HCHB EMR experience - Preferred.
Education Bachelor of Science in Physical Therapy from an American Physical Therapy Association (APTA)-approved program - Required.
