Ardor Health Solutions is seeking an experienced Travel Physical Therapist for a rewarding Home Health assignment in Griffin, Georgia! Perform comprehensive physical therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders and patient goals. Help patients improve mobility, strength, balance, safety, and functional independence.
Educate patients and caregivers on home exercise programs and fall prevention. Communicate patient progress and needs with physicians and the interdisciplinary care team. Complete accurate and timely clinical documentation.

Requirements Active Georgia Physical Therapist license (or eligibility to obtain). Home Health experience preferred. OASIS experience preferred.
Current BLS certification. Valid driver's license and reliable transportation. Strong communication, organization, and clinical skills.
