Physical Therapist - PT Home Health | Silverdale, WA Position: Physical Therapist - PT Home Health Location: Silverdale, WA Assignment Type: Local or Travel Contract Duration: 13 Weeks
Monday-Friday, 8:00 AM-5:00 PM, with one weekend every sixth week Workload: 30 units Position Overview Seeking a Physical Therapist to provide home health services across the Silverdale, WA service area. 30 units HH required HCHB Required North and South areas north is Bainbridge isle, Kingston, Hansville Poulsbo areas and south is Bremerton, Port Orchard and Olalla areas 32 hours guaranteed HCHB and Oasis required Required Qualifications & Skills Current Washington State Physical Therapist license - Required Current CPR certification - Required Valid driver's license - Required Reliable transportation and current auto insurance - Required 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required Previous home health/home care experience - Preferred HCHB (HomeCare HomeBase) EMR experience - Preferred Education Bachelor of Science in Physical Therapy from an American Physical Therapy Association (APTA)-approved program - Required Key Responsibilities Provide physical therapy services to patients in their homes.
Evaluate patients and develop individualized treatment plans. Deliver therapeutic interventions based on patient needs and physician orders. Monitor patient progress and adjust treatment plans as appropriate.
Document assessments, treatments, progress, and outcomes accurately. Collaborate with physicians, nurses, caregivers, and other members of the healthcare team. Educate patients and caregivers on appropriate exercises, mobility, safety, and home programs.
Travel throughout assigned North or South service areas to provide patient care.
