Physical Therapist- PT Home Health, Urbandale, IA - Local or Travel 13 weeks. M-F, 8am - 5pm, 40 hours per week. On-call info: On-call required if requested- on-call needs are subject to change based on staff and branch needs Productivity Units: 30 units

HCHB, Oasis, and HH experience required

Current State License as a Physical Therapist - Required. Proof of current CPR certification - Required. Current driver's license.
