1 year of Medicare certified Home Health experience. HCHB EMR experience preferred
Provide one-on-one care to patients in their homes in alignment with the plan of care Complete patient assessments and communicate progress to the care team Support individualized treatment plans and document care provided per assignment guidelines Collaborate with families and interdisciplinary providers to help improve patient outcomes Maintain professional, patient-centered care while promoting safety and comfort in the home setting Why Choose VyTalent Solutions?
