Registered Nurse - Case Manager

Job Details RN - Case Management at MSKCC - Other (New York , NY) Type/Specialty: RN/Case Management 1275 York Avenue Brief Description: This position is for a registered nurse case manager. The role is fully responsible for discharge planning from inpatient units. Utilization management functions are not part of this role.

Candidates will ideally have an understanding of discharge planning processes and will be comfortable with patient/family communication regarding post-acute arrangements. Knowledge of hospice and end of life care principles is a bonus. This position is fully in-person (full

attached) Shift times (8, 10 or 12 hours/ Start end time): 8 hours M-F Call
