Registered Nurse - Case Manager

**Registered Nurse Case Manager (RN CM) – Travel Assignment** Seeking an experienced **RN Case Manager** for an acute-care hospital travel assignment. **Requirements:** * Minimum **1 year of acute-care hospital Case Management experience** * Current RN license * **EPIC experience required** * Experience with **MCG or InterQual preferred** * **CCM certification preferred, but not required** * Intermediate computer skills * Strong clinical assessment, care coordination, discharge planning, and communication skills * Ability to work **four 10-hour shifts per week**, including a **rotating weekend schedule** * Ability to work independently and effectively within a multidisciplinary care team **Ideal Candidate:** A self-directed RN Case Manager who is experienced in managing complex hospitalized patients, coordinating services, facilitating timely transitions of care, and documenting accurately in the electronic medical record.
