Average daily caseload: 5 to 8 patients. Weekly productivity expectation: 30 points. o Visit types documentation travel and other activities have different point values. o Contractors typically see 5 to 10 patients per day totaling approximately 6 to 8 points. No guaranteed hours. o If patient census is low contact the Account Manager about floating to a nearby clinic for additional hours.
Case types may include: o Start of Care o Wound Care o Follow-up visits o Other adult home health or hospice services Minimum
one full year of recent adult home health or hospice experience depending on specialty. First-time travelers are not accepted recent specialty experience is required. Required
CPR and BLS. Weekend requirements and on-call responsibilities vary by location and profession discuss during the interview. Must be willing to travel up to 75 miles from the home clinic.
Reliable transportation a current drivers license and agency-required liability insurance are required. Mileage is reimbursed at the IRS standard rate. o Travel between patient homes is reimbursable. o Travel to the first patient and from the last patient is not billable. Locals are accepted at the same rate.
Pending licenses are not accepted an active license must be in hand. Typical schedules: o Standard: 5x8s MondayFriday o Baylor schedule at some western-territory locations: 4x10s FridayMonday Dress code: Scrubs. Charting
o All clinicians must be proficient in Homecare Homebase (HCHB) and have used it within the past six months. o Clinicians with prior HCHB experience outside the six-month window may be considered but must complete retraining during compliance. o RNs PTs and OTs must be proficient in OASIS.
