This home hospice RN position is based in Windham, New Hampshire, a welcoming southern New Hampshire community known for its peaceful residential neighborhoods, beautiful natural surroundings, and outstanding quality of life. In this role, you will provide compassionate, patient-focused hospice care in the comfort of patients’ homes. You will support individuals and their families through end-of-life journeys by managing symptoms, coordinating care, and offering education and emotional support. The role is primarily field-based, with daily travel to patients’ residences across Windham and surrounding communities.
Typical responsibilities include performing comprehensive nursing assessments, developing and updating individualized plans of care, administering medications, managing pain and other distressing symptoms, and ensuring patient comfort and dignity at all times. You will collaborate closely with an interdisciplinary team that may include physicians, nurse practitioners, social workers, chaplains, and home health aides to deliver holistic hospice care.
A typical day includes visiting several patients in their homes, documenting in an electronic medical record, communicating changes in condition to providers, and coordinating with family caregivers. You will provide hands-on care, patient and family education, and support around advance care planning and hospice philosophy. You may also participate in on-call coverage to respond to urgent patient needs after hours.
Shifts generally follow daytime hours with some flexibility required to meet patient and family needs, as well as participation in a shared on-call rotation that may include evenings, nights, weekends, and holidays. Patient caseload is structured to allow for meaningful time with each patient and family, with a focus on relationship-building and thorough symptom management rather than high-volume task-based care.
