Contract: 13 weeks Start: ASAP Guaranteed Hours Per Week: 36 Primary Location: Aspirus at Home Ironwood MI Shift requirement: 8-10hr days Weekend/Holiday: Every 3rd weekend and holiday rotation- weekend coverage could change dependent on nursing pool for needed coverage On-call: every 4th week-end and 1 day during the week. Travel
5 visits per day on average could be up to 6 . It can vary depending on the territory, patient census, staff availability, etc. Could be maximum of 150 miles in a given day but not all days.
Required Certifications upon submittal: AHA BLS, Active MI and WI license, Valid Driver's license, Minimum
2+ years home health experience, preferred. Charting
Oasis required. Epic preferred. Unit Information (Bed size, Case type, Patient ratio): Oasis admissions, all time points, episode management, dr calls and coordination of care, documentation in EMR timely, clinical competencies in wound, IV, ports, Lab draws, chronic disease management.
