07821 Practice Code: 12341 **NOTE for LHC GL coding guidance use the LHC Group Contract Labor GL Coding Guide available on the LHC Group Home Page Weekly Hour Guarantee: 32 On Call/Weekend Requirement: YES Coverage Areas: Arlington Belmont Lexington Lincoln Walham Watertown Wayland Weston Winchester Shift schedule (days start & end times): Monday - Friday 8AM-4:30PM with rotating weekends (roughly every 5 weeks) Will Milage be reimbursed between visits from first patient to last patient (Y/N)Y What are the top responsibilities for this position (Please be as detailed as possible): Initiates develops implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professionals involved in care. Directly and/or indirectly supervises care provided by the home health aides and licensed practical vocational nurses provides instruction as appropriate and assigns tasks according to State and federal regulations. Also provides required supervisory visits. Provides high quality clinical services within the scope of practice and within infection control standards in accordance with the plan of care and in coordination with other members of the health care team. Accurately and timely completes the comprehensive assessments (OASIS) including medication reconciliation. Accurately and thoroughly documents patient visits per policy and payer requirements and syncs timely per policy. Must have qualifications/experience: Massachusetts RN License What skills/attributes are nice to have and will set a candidate apart ability to work autonomously quick learner and ability to onboard fast nursing home care experience with emphasis on post acute patients and assisted living patients.
