08:00 - 16:30; Monday - Friday; start time ranges from 8a-9a for 8.5 hours; weekends required once per every 4 weeks Contract Length: 13 weeks Candidate Type: Traveler

2+ years of acute care Case Manager experience; Non acute disciplines will not be considered; Active PA RN license; BSN

Coordinate case management activities in a hospital setting. Support patient care planning and coordination across the care continuum. Collaborate with healthcare professionals to facilitate appropriate patient care.
