Travel Occupational Therapist (OT) – Skilled Nursing Facility Lake City, Florida We are seeking a Travel Occupational Therapist for a 13-week Skilled Nursing Facility contract in Lake City, Florida . This opportunity is a great fit for an OT with SNF experience who enjoys post-acute rehab, geriatric care, and helping patients improve independence, safety, ADLs, and overall functional ability. Position Highlights Skilled Nursing Facility / Post-Acute Rehab 13-Week Travel Contract 30 Hours Per Week Guaranteed Occupational Therapist Adult & Geriatric Patient Population Lake City, Florida Start Date: 11/2/26 End Date: 2/2/27 Responsibilities Evaluate patients’ functional abilities and develop individualized treatment plans.

Provide skilled occupational therapy services in accordance with physician orders and facility protocols. Address ADLs, upper-extremity function, cognition, safety, mobility, and functional independence. Educate patients and caregivers on adaptive equipment, compensatory strategies, and home programs.

Collaborate with nursing staff, PT, SLP, and the interdisciplinary rehab team. Complete accurate and timely documentation and progress reporting. Requirements Florida OT license required.
