Position Highlights Skilled Nursing Facility / Post-Acute Rehab Full-Time Contract 40 Guaranteed Weekly Hours Adult & Geriatric Patient Population Collaborative Rehab Team Norfolk, Virginia Start Date: 9/7/26 Responsibilities Perform speech, language, cognitive, and swallowing evaluations. Develop individualized treatment plans based on patient needs and physician orders. Treat communication, cognitive-linguistic, voice, and swallowing disorders.
Provide dysphagia management and patient/caregiver education. Collaborate with nursing staff, physicians, PT, OT, and case management. Complete timely documentation and progress reporting.
