Pride Health is hiring a Registered Nurse (Home Health) to support our client's healthcare facility in the local service area. Job Details:

8-hour day shifts Location: Hiawatha, IA Float / Coverage Area: Hiawatha, IA Job Type: Contract Contract Length: 13 Weeks Mileage Reimbursement Rate (if applicable): Yes. Pay offered is based on experience, expertise, credentialing, and education. Duties: Conduct comprehensive home health assessments and develop individualized patient care plans.

Implement, document, and evaluate the Plan of Care (POC) while ensuring quality and continuity of care. Provide skilled nursing services, including wound care, IV therapy, catheter care, medication management, and patient education. Coordinate care with physicians, interdisciplinary teams, patients, families, and community resources.

Maintain accurate clinical documentation, ensure regulatory compliance, and monitor patient outcomes.

Graduate of an accredited associate degree in nursing (ADN) or Nursing Diploma Program required. Bachelor of Science in Nursing (BSN) preferred. License(s): Active Registered Nurse (RN) license in the state of practice required.

Valid driver's license required. Reliable transportation and auto insurance required. Certification(s): Current CPR/BLS Certification required.

CHPN (Certified Hospice and Palliative Nurse) certification preferred. Years of

1-2 years of RN experience in a clinical care setting required. 2+ years of Home Health experience required.
