Required

Bachelor’s Degree in Physical Therapy (PT)Required Years of

minimum 1 year Required Licenses &

Certified Home Care experience (Part A) with OASIS knowledge and current usage. EPIC preferred. Registered Physical Therapist.

BLS must be AHA. Note: We need a PT who already has certified (Medicare) home care experience and knows how to complete an OASIS assessment. Ideally, we also want someone who knows how to use EPIC documentation.
