Therapy/Rehabilitation - Occupational Therapist School

OT: 4 - positions Full Time 7.5 hours per day 37.5 hrs. per week In – person only (no tele) BR: $90-130 BOE Grade Band will depend on candidate vs. needs. Caseload: 48 South Kitsap- Beautiful town on the peninsula. Surrounded by water. 30 min drive to Tacoma and 30 min Ferry to Seattle Certificates and Licensing: Current OSPI ESA Confirmation BEFORE interviewing and required before starting **** Please fill out and return when contract is signed.

Required to complete onboarding paperwork to enter our system BEFORE first day. Can not start until completed and entered in our system (attached): SKSD HR Contractor Form Sexual Misconduct Release Form Please have the contractor fill out the form down to where they sign and date. Please fill one form out for each school district that they have worked at.

Then, return the form to me and HR will send it off to the address listed in the box at the top of the form, for them to complete the bottom section of the form. If there is no previous school district employment – just note that. Provide Copy when contract is signed.
