Qualifications Active Massachusetts Physical Therapist (PT) license required. Valid driver's license required. Graduate of an accredited Physical Therapy program.
Minimum of one year of clinical physical therapy experience under qualified supervision. Home health or community-based physical therapy experience preferred. Strong clinical assessment, treatment planning, and problem-solving skills.
Excellent communication and interpersonal skills with the ability to effectively educate and support patients, families, and caregivers. Ability to work independently and manage patient care in a home-based setting. Proficiency with electronic health record (EHR) systems for clinical documentation and communication.
Ability to collaborate effectively with physicians and members of an interdisciplinary care team. Responsibilities Conduct comprehensive physical therapy evaluations and develop individualized, goal-oriented Plans of Care based on patient needs and clinical best practices. Provide skilled physical therapy services independently in patients' homes as part of an interdisciplinary care team.
Develop and implement treatment programs to improve mobility, strength, balance, endurance, functional independence, and safety. Manage an assigned caseload in accordance with agency productivity and quality standards. Monitor patient progress and modify treatment plans based on functional gains, clinical findings, and barriers to achieving established goals.
Educate patients, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, use of assistive devices, and home safety. Coordinate patient care with physicians, caregivers, and interdisciplinary team members to promote continuity of care and positive outcomes. Complete timely and accurate clinical documentation, including evaluations, visit notes, progress reports, care coordination, telephone communications, and physician orders.
Perform and document formal and informal case conferences as appropriate. Supervise and coordinate services provided by Physical Therapist Assistants (PTAs) in accordance with applicable regulations and agency policies. Recommend appropriate durable medical equipment and assistive devices based on patient needs.
Support improvements in key quality measures, including mobility, fall prevention, functional independence, pain management, and hospitalization reduction.
