Develop implement and modify individualized treatment plans to improve mobility strength balance safety and functional independence. Complete documentation accurately and timely in accordance with agency state federal and Medicare guidelines including OASIS assessments as applicable. Manage a home health caseload and coordinate care with physicians nurses occupational therapists speech therapists medical social workers and other interdisciplinary team members.
Participate in case conferences and staff meetings to discuss patient progress and care planning. Educate patients caregivers and family members on disease management home exercise programs fall prevention and safe mobility techniques. Communicate changes in patient condition to the care team and physician as appropriate.
Maintain productivity expectations of 30 points per week Must have qualifications/experience: Licensed Physical Therapist (PT). Home health case management experience. OASIS experience.
Valid driver's license reliable transportation and current auto insurance. Strong clinical assessment and documentation skills. What skills/attributes are nice to have and will set a candidate apart Previous Homecare Homebase (HCHB) experience.
Strong time management and organizational skills. Excellent communication and patient education abilities. Ability to work independently while collaborating effectively with an interdisciplinary team.
Experience managing a diverse home health patient population. Type of certification required: Current Physical Therapist (PT) license in the state of Idaho. Current CPR certification preferred or required per agency policy.
Idaho department of health and welfare fingerprinting Shift start & end times: 30 points per week - Business hours: 8:00 AM 4:30 PM. Productivity expectation: 30 points per week. Work days per week: Monday through Friday 5 days per week unless productivity expectation not met then Saturday may be needed Dress code/scrub color: Professional scrubs or business casual attire.
