Perform Skilled Patient Evaluation and Establish the Plan of Care A Home Health PT must conduct an inhome physical therapy evaluation to assess functional mobility strength balance pain and safety. Based on this assessment the PT establishes or contributes to the physicianordered Plan of Care (POC) including goals frequency and skilled interventions. This responsibility is essential for Medicare coverage and clinical appropriateness. 2.

Provide Skilled Physical Therapy Treatment The PT delivers medically necessary skilled physical therapy services in the patients home. This includes therapeutic exercise gait and balance training transfer training functional mobility pain management and fall prevention. Interventions must be tailored to the patients home environment and focused on improving or maintaining functional independence. 3.

Complete Required Documentation and Communicate With the Care Team (RN. PCM BOM PTA ST OT ED/RN) Accurate timely documentation (commonly including OASIS components visit notes reassessments and discharge summaries) is required for regulatory compliance reimbursement and continuity of care. The PT must also communicate patient status and progress with physicians and interdisciplinary team members to ensure coordinated safe care.
