Responsibilities include: Perform comprehensive physical therapy evaluations and ongoing reassessments identifying impairments functional limitations safety risks and need for skilled interventions. Develop implement and revise individualized plans of care focused on improving mobility strength balance endurance pain management and functional independence. Provide skilled interventions including therapeutic exercise gait training transfer training balance activities stair training and home safety education.

Educate patients and caregivers on home exercise programs fall prevention use of assistive devices energy conservation and safety strategies. Accurately and timely document evaluations visits progress notes and discharges using the agencys electronic medical record (EMR). Coordinate care with physicians nurses occupational therapists speech therapists and home health aides to ensure continuity and quality of patient care.

Identify and promptly communicate changes in patient condition safety concerns or barriers to progress to the care team and physician. Participate in care coordination case conferences and interdisciplinary collaboration as assigned. Adhere to all CMS Conditions of Participation infection control standards and agency policies.

Manage a mobile caseload efficiently while meeting productivity and documentation expectations. Serve as a positive shortterm clinical resource adapting quickly to new environments territories and workflows. Must have qualifications/experience: Graduate of an accredited Physical Therapy program.

Current Physical Therapist license in the state of assignment (Florida if applicable). Minimum of 1 year clinical experience as a Physical Therapist home health experience strongly preferred. Experience providing skilled therapy to older adults and medically complex patients.

Ability to work independently in the field with strong clinical judgment. Proficiency with electronic documentation systems and mobile technology. Valid drivers license reliable transportation and active auto insurance.

Willingness to travel within the assigned coverage area and adapt to different branch operations. What skills/attributes are nice to have and will set a candidate apart Prior home health travel assignment experience. Strong knowledge of Medicare Part A home health regulations and skilled documentation requirements.

Excellent time management and organizational skills in a fastpaced mobile environment. Ability to quickly build rapport with patients caregivers and interdisciplinary team members. Flexible adaptable and solutionoriented mindset.

Strong communication skills with both clinical and nonclinical staff. Experience mentoring or informally supporting staff during shortterm assignments. Comfort working with highacuity or highrisk patient populations.

Demonstrated focus on patient safety fall reduction and hospitalization avoidance. Type of certification required: Active Physical Therapist license in the state of assignment. Current CPR/BLS certification.

Valid drivers license and compliance with agency background and onboarding requirements.
