NO NEW GRAD - M-TH 8-5, Fri 7-12 811164 NPI: Please request the candidates Physical Therapist NPI. The NPI (National Provider Identifier) is required in order to proceed with an offer if the candidate is selected. 2 patients per hour either concurrently or in alternating 30-minute sessions evaluations are 1 hour.
