Therapy/Rehabilitation - PT Outpatient

NPI: Please request the candidate’s Physical Therapist NPI. The NPI (National Provider Identifier) is required in order to proceed with an offer if the candidate is selected. •

• on the hour, 8 a.m., 9 a.m., etc. New patients are scheduled individually; follow-ups are scheduled two per hour.
• No techs; expectation is 50–55 visits per 40-hour week, averaging 10 visits per 8-hour day.

• Locals accepted
• Experience REQUIRED: 1 year preferred (will consider <1 year with outpatient or SNF experience)
• Outpatient experience preferred
• Assignment Length: 13 weeks (26 weeks highly preferred)
• EPIC EMR REQUIRED
• Valid State License REQUIRED (must be in hand; check with AM if license pending)

• BLS preferred (copies required prior to start; certifications in hand preferred)
• Orientation: 1 day
• RTO: Max 10 days (facility flexible depending on staffing; confirm with AM)
• Holiday Requirement: 30 hours worked during holiday weeks
• Block Booking: Possible depending on staffing levels
• Start Date: 2–3 weeks preferred (aligns with compliance timeline)
• Interview Process: 15-minute phone screen; manager conducts direct call and text
