Therapy PTA Home Health HUMJP00040468 Average # of patients/day: 4-6 patients per day. Points per day/week: How are points broken down On average 24-30 points per week Guaranteed Hours: Varies from facility to facility. Types of Cases: Home Health Wound care medication administration etc.

Years of experience REQ: Min 1 year Home Health 2 years experience overall as a clinician. First-timers accepted: (Y/N) Yes. Weekend REQ: Varies from facility to facility. - *Have the travel ask about On Call during Interview (Discuss upon interview) Certs REQ: BLS and valid License in hand.

Is on-call REQ (Y/N) Must be open to occasions On call rotations discuss upon interview Territory/County covering: Must be willing to travel up to 50 miles outside home base Mileage reimbursed .38/mile (what we're able to bill AYA will pay IRS standard) - not to first patient or back home Locals accepted: (Y/N) What is the local radius rule Yes 50-mile radius for locals. Special

HCHB & Oasis required for RNs and some PTs depending on facility. HCHB required for all but if the traveler does NOT have HCHB training the travelers MUST complete the HCHB training at a cost of 250.00 that they must pay upfront. The traveler can be reimbursed but the client cannot be billed for this training and proof of certification MUST be provided before traveler starts or this will result in a cancel.

Dress Code: Scrubs any color Charting: HCHB & Oasis Description: The Licensed Physical Therapy Assistant assumes responsibility and accountability for the delivery of appropriate physical therapy services within a framework of Interdisciplinary collaboration and consultation.

Services are furnished under the supervision of a qualified Licensed Physical Therapist.

1. Administers the physical therapy treatment plan as developed by the Physical Therapist. 2. Administers non-complex active and passive manual therapeutic exercises therapeutic massage traction heat light cold water and electrical modalities to patients. 3.

Accurately documents daily weekly or monthly records of clients' status. Submits clinical paperwork to the office on time each week. 4. Attends educational meetings and coordinates and contributes to department in-services. 5.

Notifies physical therapist Clinical Manager or Branch/Clinical Director of any equipment failures lack of supplies staff or client changes and any other departmental changes. 6. Schedules clients and performs varied office tasks. 7. Applies concepts of infection control and universal precautions in performing patient care activities. 8.

Accepts clinical assignments that are consistent with education and competence to care for patients. 9. Promotes compliance with all fiscal intermediaries and/or other third party payors through education coaching and other assistance as necessary. 10. Performs other related duties as assigned or requested.

Knowledge/Skills/Abilities: 1. Uses effective interpersonal relations and communication skills. 2. Comprehensive knowledge of the physical emotional social and biological changes that occur in the geriatric population. 3.

Self-directed enthusiastic and accepts constructive feedback. 4. Maintains current licensure certifications and meets mandatory continuing education requirements. 5. Demonstrates commitment professional growth and competency. 6.

Ability to work flexible schedule and travel locally. 7. Ability to maintain confidentiality. 8. Demonstrated ability to work independent and cooperatively as an effective team member. 9.

Ability to do heavy lifting bending pulling pushing and standing. Prolonged standing and walking required. Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs. 10.

Requires hand-eye coordination and manual dexterity. 11. Must read write and speak fluent English.
