- 4:30 PM Will Milage be reimbursed between visits from first patient to last patient (Y/N) Y What are the top responsibilities for this position Provide skilled physical therapy treatments to patients in their homes per the established Plan of Care and supervising Physical Therapists direction. Implement therapeutic interventions including therapeutic exercise gait training balance training transfer training and functional mobility activities. Observe assess and document patient response to treatment progress toward goals and functional outcomes. Accurately document all visits timely in the EMR using measurable objective and individualized clinical content in compliance with Medicare and agency standards. Communicate patient status changes safety concerns lack of progress or need for Plan of Care updates promptly to the supervising PT. Follow infection control safety and universal precautions during all home visits. Educate patients and caregivers on home exercise programs fall prevention assistive device use and functional safety within the home environment. Collaborate with the interdisciplinary care team (RN OT ST case management) to support coordinated patient-centered care. Manage assigned visits efficiently to support productivity expectations visit frequency and geographic coverage needs. Maintain licensure competencies and regulatory compliance according to Florida practice acts agency policy and payer requirements.
