BONUS INFORMATION: THE BONUS SHOULD BE PRESENTED AS $3,750 PRE-TAX. THAT IS HOW IT WILL BE DISPLAYED ON THEIR CONTRACT. BANNER IS OFFERING $7,000 SO YOU WOULD PUT $7,000 INTO YOUR MARGIN.

AYA WOULD PAY TAXES ON THE $7,000 WHEN WE BILL BANNER MAKING IT $5,250 PRE-TAX TO YOUR TRAVELER. Additional Comments/Requirements: Required: SLP License in Colorado and DORA registration, BLS Cert and CO CAPS; Caseload has declined due to lack of a consistent speech therapist availability, but hours can be gained by working at our sister facility - SRMC. Would like a clinician that can perform Modified Barium Swallow Studies.
