Allied Health License for AZ or Compact as applicable Applicable recent experience in this work setting Commitment to quality care and adherence to client legal and client protocols
Doctor of Pharmacy or a Bachelor of Pharmacy degree; current valid and unrestricted license to practice Pharmacy; BLS
