This role serves as a key liaison between referring providers, patients, families, case managers, and the rehabilitation team to facilitate timely and appropriate admissions into rehabilitation services. The Admissions Coordinator evaluates referrals, gathers clinical and insurance information, coordinates the admission process, and helps ensure patients are transitioned efficiently into the appropriate level of rehabilitative care.
