Description

The Hospice Admission Coordinator, Social Worker will work with the hospital, home health, other facilities (hospitals, ALF, SNF) and hospice team to coordinate care across the continuum. The hours are mostly daylight with occasional evenings and weekends as required to meet patient and family needs.
