The Ambulatory Pharmacy Operations Pharmacist is a centralized pharmacy-based position within UF Health Jacksonville Ambulatory Pharmacy. This role focuses on medication access, specialty pharmacy services, Medication Therapy Management (MTM), transitions of care support, prescription processing oversight, medication adherence programs, and ambulatory pharmacy operations and dispensing services across multiple service lines.
Unlike embedded clinic-based ambulatory care pharmacist positions, this role is primarily located within the ambulatory pharmacy practice setting and is not assigned to a specific clinic. The pharmacist collaborates closely with a multidisciplinary team that includes ambulatory pharmacy operations pharmacists, specialty pharmacists, embedded clinical pharmacists, pharmacy technicians and analysts, providers, nurses, and care coordinators to support comprehensive medication management and patient access initiatives.
Pharmacists in this role utilize Epic, a leading integrated electronic medical record (EMR) system, to review patient records, laboratory values, medication histories, provider documentation, and care plans. The EMR serves as a critical tool for identifying medication-related problems, supporting specialty pharmacy services, coordinating care, facilitating medication access, documenting clinical interventions, and optimizing patient outcomes across the continuum of care.
Key responsibilities include medication access services, prior authorizations, specialty and specialty-lite pharmacy program support, MTM, medication reconciliation, adherence interventions, transitions of care activities, prescription fulfillment oversight (including medication preparation, clinical review, verification, and dispensing activities within Epic Willow Ambulatory), refill management, shipping and delivery coordination, quality improvement initiatives, and regulatory and accreditation activities.
The pharmacist serves as a resource to patients, providers, and pharmacy staff while helping optimize medication outcomes, improve access to therapy, and support operational excellence within the ambulatory pharmacy and specialty pharmacy programs. Through our 340B program, comprehensive medication access services, and coordinated pharmacy care model, we help patients overcome barriers to therapy and achieve optimal outcomes. Our specialty pharmacy program provides comprehensive support for patients receiving complex therapies through clinical monitoring, refill coordination, adherence management, prior authorizations, financial assistance coordination, and medication delivery services.
In collaboration with embedded clinical pharmacy services throughout the health system, our ambulatory pharmacy team supports continuity of care, medication optimization, and enhanced patient access across multiple outpatient specialties. Responsibilities Clinical & Patient Care Services Provide Medication Therapy Management (MTM) and comprehensive medication reviews Conduct medication reconciliation and identify, prevent, and resolve medication-related problems Provide patient education and adherence counseling Monitor therapeutic outcomes and support medication optimization initiatives Assist with transitions of care and outpatient medication management Medication Access & Prior Authorizations Facilitate prior authorizations, appeals, formulary exceptions, and payer-required documentation Coordinate patient assistance, manufacturer programs, copay assistance, and financial support resources Collaborate with providers and clinic staff to improve medication access and reduce treatment delays Specialty & Specialty Lite Pharmacy Services Support patient onboarding, refill coordination, adherence monitoring, and ongoing clinical assessments Coordinate care for specialty medication patients and maintain documentation required by accreditation and payer standards Support Specialty Lite services and coordinate care Pharmacy Operations Oversee workflow associated with prescription fulfillment, refill processing, shipping, and medication delivery services Ensure compliance with state, federal, payer, and accreditation requirements Support inventory management, operational efficiency, and quality assurance initiatives Provide oversight and guidance to pharmacy technicians and support personnel Qualifications Required
Bachelor's degree in Pharmacy -OR- Pharm D. degree (Doctor of Pharmacy). Preferred
Post-graduate residency training. Necessary
Attention to detail and ability to distinguish drug products. Fine motor coordination and manual dexterity. Computer skills and accuracy in calculations.
Excellent communication and customer service skills. Supervision of technician employees. Required Licensure/Certifications: Registered Pharmacist in the state of Florida -OR- Eligible for Pharmacist license in the State of Florida within 90 days of the date of hire/transfer.
