The Advanced Practice Provider (APP) Hospitalist provides high-quality, evidence-based inpatient medical care in collaboration with the supervising physician(s). The APP evaluates, diagnoses, treats, and coordinates care for hospitalized patients while promoting safe, efficient, patient-centered care. Responsibilities include managing acute and chronic medical conditions, participating in multidisciplinary rounds, facilitating discharge planning, and supporting quality improvement initiatives.
The APP practices within the scope defined by state regulations, hospital bylaws, credentialing requirements, and collaborative practice agreements.
and Responsibilities Direct Patient Care Perform comprehensive history and physical examinations. Evaluate and manage hospitalized adult patients with acute and chronic illnesses. Develop and implement evidence-based treatment plans.
Order, interpret, and act upon laboratory, imaging, and diagnostic studies. Prescribe medications and therapies within scope of practice. Perform daily inpatient rounds and document patient progress.
Respond to changes in patient condition and initiate appropriate interventions. Coordinate admissions from the Emergency Department and direct admissions as assigned. Participate in discharge planning to ensure safe transitions of care.
Provide patient and family education regarding diagnoses, treatment plans, medications, and follow-up care. Care Coordination Collaborate with hospitalists, specialists, nursing staff, pharmacists, therapists, case management, and ancillary departments. Participate in multidisciplinary rounds.
Communicate effectively with consulting physicians and primary care providers. Assist in reducing unnecessary length of stay while maintaining quality outcomes. Facilitate timely consultations and diagnostic testing.
Documentation Complete accurate, timely, and compliant documentation in the electronic medical record. Ensure documentation supports medical necessity, coding accuracy, and regulatory requirements. Complete discharge summaries and transition documentation as appropriate.
Quality and Safety Follow evidence-based clinical guidelines. Participate in quality improvement initiatives. Support hospital goals related to: Readmissions Length of stay Hospital-acquired conditions Patient safety Sepsis management Core measures Participate in peer review and performance improvement activities.
Professional Responsibilities Maintain required licensure and certifications. Participate in continuing education. Maintain collaborative relationships with physicians and clinical staff.
Participate in department meetings and educational conferences. Procedures as indicated with appropriate credentialing and approved privileges
