M (35 Hours per week) No weekends required Duties & Responsibilities Purpose of Position : Participates in planning, organizing, directing, coordinating and evaluating nursing service, health and patient/resident care programs of a Skilled Nursing Facility (SNF) setting. Actively involved in the oversight of nursing operations, ensuring effective utilization of resources, high-quality patient/resident care, and compliance with all applicable local, state, specifically NYS Department of Health (NYSDOH), federal and NYC Health + Hospitals rules and regulations and nursing standards of care to improve outcomes and the patient experience.

1. Oversees the implementation and evaluation of care delivery models for multiple units within the SNF. Monitors quality metrics/dashboards.
Ensures regulatory and accreditation alignment across departments. 2. Develops and manages operational initiatives with measurable outcomes and participates in a system of continuing evaluation of nursing service, health and patient/resident care programs. 3. Collaborates with SNF leadership staff, medical staff, administration and other departments in developing and implementing objectives, policies and standards of nursing service, health and patient/resident care programs.
Reviews and recommends evidence-based revisions, as needed, to unit-specific policies and procedures, ensuring continuous readiness for NYSDOH and/or regulatory surveys/audits. 4. Ensures adherence to infection control protocols at all times and that established protocols are followed during emergencies and declared disasters. Maintains staff preparedness through clear communication of roles, responsibilities, and required training. 5.
Assumes responsibility for developing and implementing staffing plans and placement of nursing personnel in accordance with NYS Staffing requirements. 6. In conjunction with facility leadership, participates in strategic planning, directing, organizing and evaluating nursing service, health and/or patient/resident care programs. 7. Participates in implementing an effective system of clinical and administrative nursing records and reports, ensuring compliance with legal and policy requirements while maintaining proficiency in EPIC Electronic Health Record system. 8.
Assumes responsibility for supervising assistant directors of nursing and other appropriate staff and directing the evaluation of personnel performance. Responsible for performance evaluations and disciplinary action of all assigned staff. 9. Analyzes quantitative and qualitative data regarding the patient/resident experience and staff engagement to identify trends and areas for improvement; implements appropriate interventions. 10.
Supports Director of Nursing and Deputy Director of Nursing in leading workforce planning, facilitates professional development of nursing staff, and leads strategic retention and engagement efforts. 11. Assesses nursing services through rounds, chart reviews and observation/evaluation of patient/resident care. 12. Participates in research projects, as applicable.
Develops, plans, coordinates, directs and evaluates other assigned projects and health programs. 13. Participates in operating and procedural councils and committees, as assigned. Maintains accurate and complete records related to nursing operations and services that are in compliance with all legal, regulatory, and policy requirements.
Participates in implementing an effective system of clinical and administrative nursing records and reports. 14. Partners with the Office of Patient Centered Care ensuring participation in defined initiatives, including but not limited to, shared governance, nursing residency programs, clinical ladder care, as assigned. 16.
Identifies and investigates issues or variances in practice and/or operations by participating in the Root Cause Analysis process and implements corrective action plans, as necessary; reports outcomes to executive leadership. 17. May assume responsibility for planning, directing, coordinating and evaluating nursing service on the weekend/evening and/or night tours. Provides guidance to nursing administrators to ensure continuity of patient/resident care on off tours. 18.
Performs all duties of the Director of Nursing or Deputy Director of Nursing in their absence.
1. Valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and 2. Master’s Degree from an accredited college or university in Nursing Administration or a related nursing discipline; and three (3) years of satisfactory nursing experience, two (2) years of which must have been in a responsible administrative or supervisory capacity, which may include managing a specific department or nursing unit within a hospital, overseeing patient care, staff management and the day-to-day operations of the department, as well as relevant clinical nursing experience in the area of nursing practice assigned; or 3.
Bachelor’s Degree from an accredited college or university in Nursing; and four (4) years of satisfactory nursing experience, three (3) years of which must have been in a responsible administrative or supervisory capacity described in 2 above. 4. May require Basic Life Support (BLS) through the American Heart Association (AHA) depending on assignment/health care setting. If required, BLS must be obtained within sixty (60) calendar days of appointment to the title and maintained for the duration of the assignment.
