About the role
This position is a joint position between Humboldt Park Health and The Children’s Care Foundation. Based in the ED and Ambulatory Care Services, the HPH Navigator will screen HPH patients that arrive at the ED and or clinics from any of the immigration shelters and or bring by the Police Department as well as referred to the Family Medicine clinic by our community organizations.

This position is a joint position between Humboldt Park Health and The Children’s Care Foundation. Based in the ED and Ambulatory Care Services, the HPH Navigator will screen HPH patients that arrive at the ED and or clinics from any of the immigration shelters and or bring by the Police Department as well as referred to the Family Medicine clinic by our community organizations. The Navigator will serve as a bridge between the patient, the medical system, and community-based organizations by building trusting relationship with community members served by the program.
The Navigator will also help in identifying barriers to accessing quality care, work with individuals to overcome these barriers, provide relevant referrals
