Description

Develops programs, manages services, and coordinates the quality, evidence based care of patients within the Cardiac Division at Cape Fear Valley Medical Center, The coordinator assists in the integration of services provided by physicians, Cape Fear Valley Health System, community organizations, and other healthcare providers to insure an optimum experience of care from the patient's perspective. By synchronizing care and services, the coordinator strives to insure evidence-based medicine is used in the treatment of the patient ultimately reducing 30-day readmission rates for cardiovascular patients at Cape Fear Valley Medical Center.

The coordinator: initiates and participates in patient, caregiver and staff education; actively markets the cardiovascular programs; participates with various committees as deemed necessary. The coordinator demonstrates strong leadership capabilities and must be able to motivate and empower others to achieve excellence in evidence based cardiovascular care. Major Job FunctionsExercises authority and accepts accountability for consistency and evidence based cardiac patient care delivered throughout Cape Fear ValleyLeads the Chest Pain Accreditation initiative and continually updates mandatory elements within this program.

Collaboratively leads the Disease Specific Cardiovascular Quality Program in collaboration with Cardiac Quality Medical Director and Cardiovascular Quality Advisory Committee. Leads the TAVR (Transcatheter Aortic Valve Replacement) coordinates Clinical Patient NavigationDelegates the management of processes to staffCreates a shared vision with the facility team to achieve the mission and vision of the Cardiac DivisionDemonstrates leadership skills including, but not limited to, problem solving, delegation and time management/organizationCreates an environment for change/transition and staff empowerment.

Leads multidisciplinary team efforts within the health system for the purpose of improving Cardiac Care Processes withthe AMI, NSTEMI and TAVR care processes, patient outcomes and quality benchmarking. Works in collaboration with the Senior Registry Analyst to review and analyze metrics from STS, ACC and NCDR TAVR Registries. Communication:Participates in marketing, public relations and education programs for Cardiac QualityAssists with the development of protocols, standards, and educational materials and ensures that they are current and meet the accepted national standards of care.

Provides monthly cardiovascular status dashboards to the Medical Director of Cardiac Quality, Service Line Director, Administration, and the Corporate Director of Quality and Safety. Coordination:Assists Cardiac Quality Teams in developing, implementing and monitoring systems to ensure transitions of care from acute inpatient phase through transitions of care. Leads and supports Cardiac DSC process and performance measure compliance.

Assists with data collection, analysis and· database management for Cardiac Quality projects and programs. Participates in quality monitoring through patient record review. Evaluates metrics from NCDR TAVR Registry and presents to Cardiac Quality Committee monthly.

Aligns EMR and all documents/forms to support care delivery in the areas assigned. Recommends changes and modifications to systems as required to ensure timeliness, accuracy, and efficiency for AMI, NSTEMI and TAVR care and measurement of care. Acts as liaison with physician leadership, administration, AHA Mission Lifeline, hospital personnel, as well as other interested parties to coordinate the various aspects of an indicator and outcome compliance on a regional level.

Facilitates active regional participation of all entities in the STEMI system of care per AHA Mission LifelineEncourages and supports partnering facilities in the region to conduct routine review of the following and provide feedback on Current state of Cardiac Care including AMI, NStemi, TAVR care Systematic changes likely to improve the probability and speed of therapy Education and supporting materials regarding the ACC Cardiac Care including AMI, NSTEMi, and the patient undergoing the Transcatheter Aortic Value Replacement procedure.

Patient EngagementRounds throughout the healthcare facility to begin education to assist with self-management. Rounds on all patients upon their discharge and provides business card if further questions occur once the patient has returned to home. Assures the Discharge Instructions are correct reflecting the Cardiac Diagnosis.

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:Education and Formal Training: Bachelor of Science in Nursing requiredCurrent licensure in North Carolina as a Registered Nurse or Compact state licensureNurse Practitioner preferred and encouraged to applyWork

2 years’ experience in working with Quality/Patient Safety and Regulatory, Quality Process and/or Data AnalysisCath lab, CSICU, or CSOR experience preferred. Other related experience, such as Cardiovascular Device care also considered. Experience with clinical trial management, quality and data abstraction or clinical research coordination experience a plus.

Previous supervisory/leadership role preferred. Knowledge, Skills, and Abilities Required: Proficiency in reading, writing, and speaking the English languageKnowledge of PI methodologies, Joint Commission and CMS standards, and patient safety issuesKnowledge of quality planning, cost of quality, and importance of documentationKnowledge of basic statistics, distributions, probability, and measurement scaleSkills with the audit process to include audit types as well as audit roles and responsibilitiesDemonstrates positive interpersonal skills.

Verbal and written skills required. Demonstrates effective organizational and leadership skills. Is knowledgeable of the age specific physiological, psychosocial, developmental and spiritual needs of the patients typically served.

Skills in working with teams to include team building techniques and understanding team roles and responsibilitiesAbility to collect, aggregate and review data. Experience with regression and correlation, hypothesis testing, design of experiments (DOE) preferredPhysical
