Job OverviewProvides clerical and administrative support to staff and faculty, by managing their phone calls and calendars, completing correspondence, maintaining files, creating reports, and scheduling complex appointments. Assists Care Coordinators and specialty providers with daily activities by managing clinic schedules that meet the needs of their patients, obtaining patient records, assisting with pre-visit planning and entering data into electronic medical records as directed.

This position will work most closely with the RN Care Coordinator(s), starting initially with a specific population and then broadening into other related areas. Administrative ResponsibilitiesCompletes the Care Coordination process for clinic team’s patients. Referrals, hospital discharge coordination, working with providers to complete Care Plans as necessary, creating goals with patients and follow-up with the patients on goal setting, and other duties for care coordination.

Liaison between clinic and community partners for shared initiatives, and long term collaborations. Assists with population health management through working panels and by addressing action items created in panel management meetings and from panel review. Completes all referrals and referral tracking for patients enrolled in care coordinationPartners with patient/care-giver/management in care/decision making.

Individualizes care/service plan to meet patient/customer needs. Modifies interventions/service based on population served. Provides patient/employee education based on assessment of learning needs of learner.

Facilitates complex health referrals. Coordinates care for patients requiring referrals..
