Care Manager LCSW at Providence St. Joseph Hospital Orange in Orange, CA. This position is Part time and will work 8-hour, Day shifts.
The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Responsibilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management LCSW is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral or legal barriers to accessing necessary resources and care.

Master's Degree in Social Work from an Accredited Council on Social Work Education program Upon hire: California Clinical Social Worker License 2 years SW experience in any healthcare setting Clinical experience in a healthcare setting. 1 year Experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program
Basic understanding of acute medical surgical healthcare terminology, prior case management experience
