Develop and manage individualizedDevelop and manage individualized discharge plans aligned with patient goals, clinical needs, and available resources
Coordinate post-acute services andCoordinate post-acute services and community resources, including home health, skilled nursing, rehabilitation, hospice, and outpatient care
