Qualifications
✓Involved with Education / PI Regional Manager in development and implementation of educational material and programs for health care team, patients, families/caregivers, and community.
✓Pt / CG education of disease management and skilled care in home environment.
✓Must demonstrate knowledge of the principles of growth and development over the life span and skills to provide care to neonatal, pediatric, adolescent, adult, and geriatric patients
✓Attends & participates in clinical conferences, staff meetings, and educational activities
✓Demonstrates initiative to obtain more than minimal expectation for continuing education
✓Must possess the ability to maintain a good working relationship with the patient, caregiver and medical community
✓Interacts daily with Home Health / Hospice staff, area physicians, vendors and auxiliary resource persons
✓Projects a positive image of NMMC Home Health / Hospice to patients, families, caregivers, and community
