Case Management Assistant

Customer-driven quality service approach. The Case Management Assistant will implement the plans and interventions defined by the Case Managers and Clinical Social Workers. Under the case management staff's direction, performs multiple activities that support the movement and transitions of patients out of the inpatient hospital setting. Assists in administering Case Management functions include organizing workflow, communications, links to community resources, transportation, and other duties to help facilitate discharges. Provides clerical and support functions to the case management staff members with the goal of promoting expeditious patient transfers and/or discharges. • Term Care, and LTACH placements) and communicates information between the hospital, payor, and post-acute facilities. Coordinates and provides documentation for patient transport to and from hospital and appointments Responsible for dedicated ambulance transportation). Arrange transportation, provide documentation, track data, and follow up to confirm successful transport. Maintains multiple databases to track patterns and identify potential discharge barriers to facilitate a timely discharge. Prepares discharge packets to be sent with patients being discharged to skilled facilities. Promote customer satisfaction through effective communication and Language of Caring skills.

High School Diploma or GED. 1-2 year’s related experience. Bachelor’s degree preferred. Medical terminology knowledge preferred.

This position has a hiring range of USD $18.70 - USD $32.72 /Hr.
