Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards. Position Details:Minimum Required QualificationsEducation:High School Diploma or equivalentLicensure:NoneExperience:1-3 years CHW experience or equivalentCertification/Registration:Mercy provided Community Health Worker training certificate within 6-12 monthsBasic Life SupportOther Skills and Knowledge:Strong relationship and collaboration skills.

Excellent communication skills. Ability to work independently. Compassion for poor and underserved members of the community.

Must be proficient with Microsoft Applications and be able to document in an EMR. Must have access to reliable transportation.
