The Community Inake Coordinator is responsible for assessing, planning, and developing referrals. 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating Weekends and Holidays Great Healthcare benefits, and education reimbursement of $5,250 annually Job Details Our Community Intake Coordinator: Coordinate referrals and facilitate smooth transitions to home care services Build and maintain strong relationships with physicians, discharge planners, and referral sources Serve as a key ambassador for Catholic Home Care within the community Identify opportunities for program growth and service expansion Deliver exceptional customer service and support to patients, families, and healthcare partners Collaborate with interdisciplinary teams to ensure positive patient outcomes
Strong relationship-building, communication, and customer service skills Experience in healthcare marketing, referral development, or business growth is a plus Passion for patient advocacy and care coordination Posted Salary Range USD $60,000.00 - USD $90,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
