You’ll need
Education
✓Bachelor’s Degree in Social Work, Counselling, Public Health, or related field, required.
Master’s Degree, preferred.
Qualifications
✓Ability to develop and implement an integrated care coordination model across TCFSH programs.
✓Ability to establish standardized workflows, referral pathways, and care management protocols.
✓Strong verbal and written communication skills, with the ability to clearly convey information to team members, healthcare providers, and families in a professional and tactful manner.
✓Ability to coach team members through change, provide constructive feedback and recognition, delegate tasks effectively, and make informed decisions.
✓Ability to foster collaboration and build positive working relationships within the team.
✓Ability to effectively interact with individuals from diverse age groups and backgrounds.
✓Strong organizational skills with a high level of attention to detail.
✓Ability to ensure services are trauma-informed, survivor-centered, culturally responsive, and equitable.
✓Ability to promote continuous quality improvement and evidence-informed practices.
✓Proficiency in Microsoft Office applications, including Outlook, Word, PowerPoint, Excel, and Teams.
✓Three years of experience in care coordination, case management, or a related human services field, required.
✓Demonstrated experience working with individuals impacted by trauma, family violence, or complex social needs.
✓Experience in healthcare, child welfare, domestic violence, or community-based settings.
Licensure (e.g., LSW, LISW, LPC, LPCC), preferred.
Prior supervisory or team lead experience, preferred.
Experience with Electronic Health Record (EHR), preferred.
