About This Role The EMS / Mobile Integrated Health (MIH) RN Program Coordinator is responsible for the day-to-day clinical and operational coordination of the NEK MIH Program. The Coordinator manages enrolled patients from intake through completion of services and serves as the primary liaison among patients, EMS field teams, NVRH, medical directors, Caledonia Home Health & Hospice, Community Connections, and other community partners.
This position requires an active Vermont Registered Nurse (RN) license and uses professional nursing knowledge and clinical judgment to review patient information and field documentation, identify clinical concerns and escalation needs, coordinate follow-up, and support field teams within established medical direction and protocols. Key Responsibilities 1. Patient & Field Operations Review new patient referrals, clinical information, medications, care goals, and program eligibility.
Coordinate and schedule MIH visits and assign appropriate field teams based on patient needs and available resources. Maintain the program census and monitor patient progress, visit frequency, graduation, escalation, and disenrollment. Coordinate services with Caledonia Home Health & Hospice and other care partners.
Provide field teams with relevant patient information before visits and review visit documentation afterward. Identify clinical concerns requiring Medical Director notification, ED evaluation, emergency response, or other follow-up. Maintain effective operational relationships with participating EMS agencies. 2.
Clinical Coordination & SDOH Serve as a clinical and operational resource to field teams within RN scope and established medical control protocols. Coordinate referrals to Community Connections for identified social determinants of health needs and track resolution. Ensure appropriate follow-up and loop closure for clinical and social needs. 3.
Quality & Program Management Co-lead program quality assurance with the Program Medical Director. Monitor documentation quality, clinical outcomes, utilization, adverse events, and other program measures. Support adverse event review and implementation of corrective actions.
Maintain current protocols, standing orders, workflows, and field team communications. Provide data and documentation for VDH RHTP grant reporting. Participate in program development, coordination meetings, process improvement, and ongoing program evaluation.
Required Qualifications Current, unrestricted Vermont RN license — required. Minimum three years of clinical experience in emergency nursing, EMS, critical care, community-based care, or a comparable clinical setting requiring independent clinical judgment. Experience with patient care coordination and multidisciplinary collaboration.
Strong clinical assessment, communication, organizational, and problem-solving skills. Ability to manage multiple priorities independently and maintain accurate documentation. Proficiency with EHR and electronic documentation systems.
BSN preferred; equivalent education and relevant clinical experience may be considered. Valid driver's license. About NVRH Northeastern Vermont Regional Hospital is located in St.
