About the role

This position supports the Care Transitions-Home Visit team/program. The care coordinator collaborates with the patients care team, which may include patient, caregivers, physicians, and community resources. This position will assist in the enrollment of moderate to rising risk members through, face to face interactions in the inpatient setting.
