Be the Best Admissions Manager You Can BeIf you meet these qualifications, we want to meet you!Associates Degree, PreferredHospice, home health, or case management experience1+ years of supervisory experience or AccentCare tenureExcellent written and verbal communication skillsAbility to work closely with a wide variety of stakeholdersResponsibilities:Ensure all patient referrals are received in a timely and complete mannerCoordinate with staff to ensure responsibility for the initial determination of a patient and their families service needsSupervise communication with providers, attending physicians, other appropriate hospice staff members, and any other agencies as needed to coordinate initiation of hospice servicesMaintain regular communication with the Director
Business Development (DBD)Assist callers with information about other community resources when hospice is not appropriateMaintain referral statistics and provide reports on a monthly basis to DBDBuild and maintain close referral and third-party payor relationships through appropriate and timely written and phone communicationMaintain confidentiality of sensitive informationCoordinate with Admissions Department, personnel assignments to cover admissions needs and assessments in an effective and timely mannerWork cross-functionally with Director
Clinical Operations (DCO), Team Directors (TD) and DBD to ensure promised services are providedParticipate in hospice orientation and ongoing education programsParticipate on committees and in special projects, as assigned, including those related to fiscal responsibilities Ensure all data is accurately entered into computer systems within guidelinesActively participate in short and long-term planningInitiate creative ways to meet the needs of patients and familiesServe as point of contact for Hospice Nurses with admissions responsibilities – including clinical skills oversight, assignments and monitoring compliance regulationsCompletes and provides 90-day and annual performance reviews for direct reportsMonitors compliance of all regulatory requirementsProvides follow-up and resolutions for both internal and external concerns and complaintsOther duties as assignedOur Investment in YouCaring for others starts with caring for you.
