  • 6p, Schedule will be rotating based on business needAssesses patient’s physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records, and clinical data.Evaluates patient’s response and reports to responsible provider to ensure optimal patient outcome.Contributes to the development and implementation of patient care plans in collaboration with the interdisciplinary team.Demonstrates competency in therapeutic and diagnostic interventions, including vital signs, minor treatments/procedures, prevention services, pre-visit planning, post-visit follow-up, and medication refills per protocol.Promotes, advocates, and collaborates to protect the health, safety, and rights of each patient.Provides patient and family education through assessment of health literacy, barriers to learning, readiness, needs, and ability.Encourages patient engagement in healthcare partnership using motivational interviewing, evidence-based self-management support protocols, tools, and teach-back methodology.Aids in the coordination of the patient’s care plan.
