About the role

Duties Description Are you ready to start your Social Work career with New York State? Creedmoor Psychiatric Center is recruiting for a Licensed Master Social Worker to provide professional social work services to individuals with mental illness. Candidates from outside or within State Service can apply. As a Licensed Master Social Worker, you will provide professional social work services to individuals with mental illness including:
Functioning as the primary therapist for a clinical caseload; including working collaboratively with multidisciplinary treatment team to complete initial assessments, treatment planning, providing treatment, and discharge planning.
Providing individual, group, and family therapy as well as crisis interventions as needed.
Facilitating thorough discharge planning and complete discharge follow-up including outreach to outpatient programs and residential programs.
Advocating for youth and families to obtain community-based linkages with additional support services to meet their needs.
Participating in facility initiatives and committees.

If appointed, you may be required to become an enrolled Medicare provider; obtain and provide to your employer a National Provider Identifier (NPI) number issued by the National Plan and Provider Enumeration System (NPPES); and otherwise actively participate to the degree necessary to allow for your services to be billed through Medicare and Medicaid. If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.
