We are looking for a compassionate Licensed Practical Nurse to join our growing Portland team. This full-time position serves patients throughout Portland and Cumberland County . What You’ll Do Provide skilled nursing care in patients’ homes under the direction of an RN and physician Follow individualized plans of care Monitor patients and report changes in condition Administer medications and treatments within your scope of practice Educate patients and caregivers Complete timely and accurate clinical documentation Promote patient safety and independence Collaborate with nurses, therapists, physicians, and other members of the care team Participate in team meetings and the on-call rotation Why Choose Elara Caring?

Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Ability to travel between patient homes throughout Portland and Cumberland County The base salary for this position is $35.00 to $40.00 per hour, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
